safely: “As a Care Manager, it is my responsibility to provide guidance and assistance to
people to help them remain living at home for as long as possible.
The following are some tips I have picked up along the way to make sure this happens:
1. Regular health monitoring and coordination with healthcare providers
ensure that any changes in health status are promptly addressed. This proactive approach can prevent minor issues from becoming major
concerns.

2. A balanced diet and regular exercise are vital for maintaining health and
mobility. Simple exercise routines, tailored to the individual’s abilities, and
a focus on nutritious meals can make a substantial impact.
3. Regular safety assessments are crucial in identifying and mitigating
potential hazards within the home. Simple steps such as ensuring
adequate lighting, removing tripping hazards, and installing smoke
detectors can make a significant difference.
4. Providing support and resources for family caregivers is equally important.
Preventing caregiver burnout ensures that loved ones receive consistent,
high-quality care. Access to respite care and support groups can make a
big difference.
5. Assistance with daily activities such as grooming, meal preparation, and
medication reminders can help people maintain their independence.

If you are someone who does not have a support network in place, and feel as
though you could benefit from home care services, there are some basic steps to
take:
1. Call My Aged Care on 1800 200 422.
2. Register your details with the operator and request an in-home
assessment.
3. My Aged Care assessor WILL visit and review living situation, medical
conditions and provide advice on services that can be implemented to
assist.
4. Commonwealth Home Support Program (CHSP) may be awarded which will
provide entry level and basic in nature services to assist.
5. If the individual’s care needs are high enough or of a more complex
nature, a referral to the Aged Care Assessment Team (ACAT) may be
given, which is specifically for a higher level of funding, namely a Home
Care Package (HCP).
6. The Aged Care Assessment Team (ACAT) assessor to visit and conduct a
follow up in-home assessment. If assessor deems a higher level of care is
required, a Home Care Package may be given.
7. Care recipient will be placed in the national queue and wait anywhere
from 3-12 months for Home Care Package to be ‘assigned’.
8. Care recipient to choose a provider and commence care services.
